Clicking the 'Enter Feedback' button will bring up the feedback form:
Here the user simply selects the type of feedback, the location/asset related to the feedback (which is optional) and the text of the feedback itself.
They can also select whether they want to be contacted, which can be used by the supervisor once it's accepted.
To make it easier for people to enter feedback, you can also create a QR code and shortened URL to access the feedback page. To do this, just click the QR code icon in the top left of the screen with the form open.
When creating that QR code shortcut, you can also pre-select locations or assets, or types, so they don't have to select those. For example let's say you wanted to have a poster at the entrance of a machine room that says something like "See a Safety Risk? Contact Us". You can then open the Enter Feedback form, select the machine room location, select "Safety-Related" as the type, and then click the QR code icon. This will generate a QR code and short URL specifically for entering feedback for that location and type.
You can create QR code links to provide feedback for many different locations or assets in your organization, streamlining the feedback collection process.
